Must be completed by everyone who wants to volunteer or participate in any of our youth programs. Each person must complete their own form. This must be accepted by HR prior to completing any of the other tasks.

Must be completed for anyone who is interested in volunteering for any of our programs. *Once the Volunteer Application is received and approved, HR will contact the applicant and provide Livescan Forms and information. *If a minor (under 18 years of age), please request the Parental Consent Form when contacted.

If applicable, must provide results from recent TB test (must be within the last 4 years).

Expectations of all individuals working with youth programs

All Volunteers working with youth are highly recommended to complete Mandated Reporter Training each year. You are welcome to submit copies of training that you have already completed or complete the training in the link below. Please send copies of the certificate to the P&R office.
