Employee Leaves

Family and Medical Leave Act (FMLA), California Family Rights Act (CFRA), and Pregnancy Disability Leave (PDL) are leave entitlements to allow employees to balance work and family life by taking reasonable leave for specific purposes. Under Federal and State law, eligible employees are allowed to take up to 12 weeks (480 hours) of unpaid leave within a 12-month period to bond with a newborn or adopted child, to care for your own illness or injury, or to care for a seriously ill child, parent, or spouse.

WHAT STEPS DO I NEED TO TAKE TO REQUEST A LEAVE OF ABSENCE?

 Employees attempting to take leave are responsible for:

  • Provide notice to Manager/Supervisor and Human Resources that a need to take leave exists.
    • If the leave is foreseeable, must provide 30 days' notice
    • If the leave is unforeseeable, must notify Manager/Supervisor and Human Resources as soon as is practical
  • Let your Supervisor/Manager and Human Resources know if the leave will be taken intermittently or on a reduced leave schedule basis, when you plan to go on leave, and about how long your leave will be.
  • Fill out, sign and date a Request for Leave of Absence Form.
  • Have the treating health care provider fill out the Certification of Health Care Provider form for leave due to serious medical condition.
  • Submit the Request for Leave of Absence Form to your department and have your Manager/Supervisor sign and date the form.
  • Submit your Certification of Health Care Provider and Request for Leave of Absence form to the Human Resources Department via email at HR@cityofgalt.org.

Please note FMLA and CFRA are unpaid  leaves, so you will need to contact the State of California to inquire about State Disability. SDI is administered through the Employment Development Department (EDD). It is a partial-wage replacement insurance plan for California workers mandated by the state and funded through employee payroll deductions.

Military Leave

Employees who are members of the U.S. Armed Forces are eligible for military leave of absences. These documents outline the policy and instructions on requesting leave.

  • Employee may be eligible for salary continuation once sick leave and PTO has exhausted. 
  •  EDD State Disability Insurance (SDI)

 Up to 12 weeks protected leave

Subject to Caps
